NDIS Support Coordination is a capacity building support designed to help participants understand, organise, and use their NDIS plans effectively. It’s not about providing direct care—it’s about connecting you with the right services, managing those services, and helping you build the skills to do it independently over time.

There are three levels of Support Coordination under the NDIS:

(1) Support Connection – Short-term support to connect with providers

(2) Support Coordination (Level 2) – Ongoing assistance to manage service delivery

(3) Specialist Support Coordination (Level 3) – For participants with complex needs or high-level risks

SS Disability Care currently offers Level 2 Support Coordination, working closely with participants, carers, and service providers to streamline support and make sure your funding works in the real world.

Not every NDIS participant automatically receives Support Coordination—it depends on your situation and the goals in your plan.

Support Coordination is usually funded if you need help managing complex or multiple supports. Which includes participants who are new to the NDIS, transitioning from a different system, or living with a disability that makes it harder to navigate services independently.

It’s typically funded under Capacity Building – Support Coordination. If you’ve received this funding, it will appear in your plan as its own category, separate from Core Supports or other therapy budgets.

You can also request Support Coordination in your plan review. If you don’t already have it but feel overwhelmed managing your supports, we can help you prepare the right evidence and reasoning to include it in your next review.

Support Coordinators don’t deliver other funded supports. Their role is to stay independent—focused on helping you coordinate your plan, not providing direct services. 

Our coordinators are highly trained professionals with extensive knowledge of the NDIS and local Gold Coast services. They stay up-to-date with NDIS changes and best practices.

Step 2 – Share your NDIS plan: We’ll review your plan to confirm if Support Coordination is funded and what level of support you’re eligible for. If you’re unsure, we’ll guide you through it.

Step 3 – Set up an initial meeting: We’ll organise a time to meet with you (and your carer, if you like). This is where we get to know you—your goals, preferences, challenges, and what you need from a coordinator.

Step 4 – Start building your support network: Once we understand your needs, we help you connect with providers, set up services, and start using your plan properly.
